The template booking form for use of premises for Christian worship (Worship Booking Form) has been prepared specifically for occasional or one-off use of Model Trust property by another Christian church or congregation for non-Methodist services and meetings for public religious worship under Model Trust 14(2A). It would typically be used for a wedding or a one-off festival service.

Before using the Worship Booking Form, please (1) ensure you have the consent of your Superintendent and the appropriate Connexional Authority being the Connexional Ecumenical Officer (a signed Schedule 14a) and (2) satisfy yourself that a booking form is an appropriate way to document this use of your premises.
(1) To obtain the required consent of the appropriate Connexional Authority, you need to complete Schedule 14a and submit it, along with the applicant’s statement of belief, to the Connexional Ecumenical Officer for consideration and approval. Details as to how to submit the form and a link to it are available from the Methodist Church Website: Schedule 14a. Please share the documents with your superintendent first so that they can consider the proposed use, ask any questions and confirm their own approval on the form.
(2) Satisfy yourself that a booking form is an appropriate way to document this use of your premises by referring to the following guidance notes:
